Mosman Council – Aged and Disability Services provides a range of services aimed at meeting the needs of Mosman residents who are frail older people or younger people with disabilities, and their carers.
We offer an Information, Referral, Support and Advocacy Service. (See Clients Rights and Responsibilities). You are welcome to provide feedback as a client; have someone speak on your behalf and be assured that confidentiality is maintained.
As of July 2015 the HACC (Home and Community Care) program for frail older people, transitioned to the Commonwealth Home Support Program (CHSP). All new referrals of frail older people (aged 65 and over) to our service will go though My Aged Care. Contact 1800 200 422 or visit www.myagedcare.gov.au.
Neighbour Day is Every Day
On March 31 Mosman Council launched ‘Neighbour Day Is Every Day’, a project that aims to tackle the social isolation of older people in our community by educating local residents and businesses about how to identify isolated individuals and what actions they can take to make a difference. Residents can call Mosman Council Community Care Team for services, advice and information. Neighbours will be encouraged to step up and engage with individuals they suspect of being isolated, for example by offering to take out the bins, do some gardening, or help out with errands. Mosman is a wonderful place to live because many residents recognise that the strong community enriches all of our lives, and “Neighbour Day Is Every Day” will strengthen the community even more.
Mosman Council offers a Community Links program to support frail, older people and younger people with disabilities living in their own home.
Support is provided by trained volunteers who assist with social support and various tasks such as shopping, outings, transport to appointments and other activities.
The Community Links program has been extended to include an Art-at-Home and Computer Aid program. Skilled volunteers help you develop or improve your drawing/painting or computer skills in your own home.
