Mosman Community Care

Mosman Council's Community Care provides a range of services for older adults, people living with disabilities and their carers living in Mosman and neighbouring areas of Cremorne and Neutral Bay. Some of our programs are funded through the Commonwealth Home Support Program. CHSP is a government initiative subsidising services to older adults needing low-level assistance to remain living independently in their own home. Mosman Council offers Meals, Transport and Social Support.

To be eligible for CHSP services, you must be registered with My Aged Care and be aged:

  • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
  • 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people), on a low income and homeless, or at risk of being homeless.

If you need assistance registering with My Aged Care call the friendly Care Team on 9978 4128 to book an appointment to come into the Centre. 

For residents over 55, we also offer a number of interesting activities at the Mosman Square Seniors’ Centre. The centre is open Monday to Friday from 9am to 5pm and is located at the Civic Centre, Mosman Square.

What can the Care Team do for you?

The Care Team is available to assist with information, referral and support for people 65 years and older. We also aim to combat social isolation and help establish and maintain social networks within the local community.

What Social Support programs we have to offer:

  • Shopping assistance/companion visits
  • Discussion Groups
  • Walking Groups
  • Carers Support Group
  • Sketch Classes for people living with memory loss
  • Music Concerts
  • Lunches at the Mosman Seniors Centre
  • Guided yoga and mediation
  • Social outings
  • Transportation to and from the centre
  • English conversation classes

To access services you must be registered with My Aged Care 1800 200 422 or visit www.myagedcare.gov.au. Alternatively, please contact the Care Team on 9978 4119 or email the Community Care Team.

Our Consultations

In early 2020, Council conducted consultations with older residents and other interested community members and service providers on ways to support an age-friendly community and the ‘Creating a Better Mosman for Over 55s’ Consultation Report is available on Council's Social Planning page.
